	Fully completed and signed and completed SBD1, 

	Fully completed and signed and completed SBD4, A false declaration will result in disqualification. Paragraph 2.4 disclosure must align with subsection 2.4.1 and directors’ information on the Central Supplier Database (CSD).

	Valid Letter of Good Standing Compensation for Occupational Injuries and Diseases (COIDA) for cleaning sector.

	At least two references above R500 000.00 (completed work and/or purchase order/s)
