							 1.  Completed SBD 4 and SBD 6.1 (Ensure that the Table on SBD 6.1 under table 4.2 needs to be fully completed and attach proof for claimed points)

2.  Initial each page and sign the last page of the attached GCC form

3.  Send Valid Tax pin

4.   Send a recent full detailed CSD report

5.  Initial each page of the specification 

6. Supplier must have 1 SG Classwork of construction
